[STC-SM] how do you indicate that text is a link?

In usability testing that my company recently performed, many of the participants said that they couldn't recognize links. Our corporate style precludes using "www." to indicate a url; we are supposed to use bold and underlined, blue text. Internal links are blue and underlined, but not bold. Tables of contents entries are links, but in one manual they are underlined and in another they are not underlined. So, how do most of you indicate that text is a link?
